Plans for your discharge begin even before you are admitted to the hospital for surgery. The objective at Memorial Hermann | Rockets Orthopedic Hospital is to ensure that your discharge is as convenient for you as possible.
We have an experienced team that includes Registered Nurses, Physical Therapists, Case Managers and Pharmacists. They are dedicated to providing you and your family with the information necessary for you to return to your daily routines as soon as possible. Post-surgical rehabilitation, equipment, home health care and nursing assistance may be needed for your return home. Our Case Managers will coordinate this for you before you leave the hospital.
