Attention Patients and Family Members:

If you have a complaint or a concern regarding patient safety or quality of care, please contact a TIRR Memorial Hermann employee or manager at any time. Your concern may be submitted either verbally or in writing. We will work with you to resolve your complaint or concern within a reasonable time frame. If your complaint or concern is not resolved to your satisfaction, please call the Guest Relations hotline at (713) 797-5959.

You may also submit a complaint directly to the Texas Department of State Health Services, Health Licensing and Compliance Division. The complaint may be mailed, faxed or you may call the Complaint Hotline:
