About Transitional Rehabilitation
University Place Nursing Center, a 5 star rated Nursing Facility by the Centers for Medicare & Medicaid Services and US News & World Report, provides a wide range of individualized care. For patients who require short-term rehabilitation for skilled nursing needs and therapy following hospitalization, University Place is pleased to offer transitional rehabilitation care. The ultimate goal for this type of care –transitioning patients back to their homes and lives.
The program is directed by an interdisciplinary team dedicated to caring for skilled nursing needs and therapy. The team includes a medical director, advisory physicians, 24-hour nursing care, a social worker and consultation from the Memorial Hermann Rehabilitation Network. Our highly-trained and caring staff provides personalized attention to all patients.
The transitional rehabilitation program at University Place provides the best specialized rehabilitation and therapies available for our patients including:
